Office Guidelines
Regular Appointments
We strongly encourage regular checkups after your initial visit. Regular checkups include a thorough cleaning and an oral exam. We know how valuable you time is, so we strive to be on time with your appointments.
Financial Guidelines
For our patients with dental insurance:
Although we are not contracted with any insurance company (we are out of network), we are happy to help you maximize your dental benefits.
Our protocol is as follows:
- Pay in full at the time of service.
- We file a claim to your insurance company.
- You are reimbursed from your insurance company, according to your policy agreement.
Option for patients of record prior to January 2008 in good financial
standing with our office; requires a current credit card to be kept on file.
- Your estimated portion is due at time of service.
- We file a claim to your insurance company.
- Your insurance company pays us according to your policy agreement.
- We debit your credit card for the remaining balance.
- You may request a receipt at any time. We keep credit card receipts on file for 3 months.
For our patients without dental insurance:
Payment is expected in full at the time of service. Please be prepared at your appointment with cash, check, or a major credit card.
If you need a payment plan, we offer two options:
Option 1: CareCredit or Citibank Health Card. A line of credit option that offers no initial payment and a range of payment terms with no interest options.
Option 2: We can split the fee into three equal payments, with the first one due at the time of service, and one each subsequent month. This requires a credit card on file.
**Please make all financial arrangements with our treatment coordinator prior to your appointment date.
